DUI/OWI PROGRAM

RISK EDUCATION INSTRUCTIONS: Classes meet on Wednesdays only. Please contact our office for a Risk Education calendar, as the 4 modules are taught in order and classes begin/end on specific days. On your first day, you will need to arrive 15 minutes early to complete admission paperwork and a pre-test. If you did not complete your Illinois Alcohol and Drug Evaluation Uniform Report at DUI Services, then you must bring a copy with you for our file. The cost of the class must be paid in full before you can take your post-test and receive your certificate of completion.

EARLY INTERVENTION & TREATMENT INSTRUCTIONS: Please contact our office to let us know when you plan to begin sessions. You may pay per group or in full up front. Your treatment costs must be paid in full prior to any documentation being released from DUI Services to you or third parties. If your DUI evaluation was completed at another agency, please bring a copy for our file. Staff will discuss with you attendance expectations and timelines for completion. All clients are expected to attend at least once weekly, unless otherwise arranged with clinical staff. If you fail to attend classes for 30 days, we will close your file. There is a $30.00 fee to re-open your file once it has been closed. Payment is required in full prior to completion of your treatment classes.

INSURANCE: If you have health insurance, please contact your insurance company to ensure you have substance abuse treatment benefits. Insurance companies do not cover Illinois Uniform Reports or DUI Risk Education classes. Co-pays are expected at the time of service and will be requested prior to your attendance at each session. Co-pays may not cover the entire cost of treatment services and you will be expected to cover the remainder should this occur.
